Dear Parents and Guardians,

Legislative changes require Texas school districts to have the forms below completed

for all students enrolled in the district to receive counseling services. Please complete

the corresponding form for your child's grade level by October 17, 2025. If you have

any questions, please call your students’ campus and request to speak with the school

counselor.
